This is the most consequential sentence in Minnesota's loan forgiveness statute, and it is written as a parenthetical nobody reads to the end of.
What the statute says
From the Department of Health's own guidelines, quoting the law: "A designated rural area is defined by Minnesota Statutes, section 144.1501 as a statutory and home rule charter city or township that is (1) outside the seven–county metropolitan area, as defined in section 473.121, subdivision 2 excluding the cities of Duluth, Mankato, Moorhead, Rochester and St. Cloud."
Two filters, not one. Outside the seven-county metro, and not one of those five cities.

They are the five places in Minnesota, outside the Twin Cities, where a specialist is most likely to be recruited. The rural programme carves out precisely the destinations that generate the most physician relocation in the state.

Mayo Clinic's home campus is in Rochester. It is one of the most recruited-to medical employers in the United States, and Rochester is named in the exclusion.
So a physician arriving at Mayo who has read "Minnesota pays $33,000 a year in rural loan forgiveness" is reading about a programme they are not geographically eligible for. The rural door is closed by the city's name being in the statute.
★ What is not closed: the urban programme does not use a city list. It uses federal designation. If a specific Rochester practice site sits in a HPSA, MUA or area with medically underserved populations, the urban programme pays the same $33,000 a year to $132,000.

And the seven-county filter
The first limb removes the seven-county metropolitan area, which is the Twin Cities and its ring. A physician in Minneapolis, St. Paul or the suburbs is not rural either, for the same reason, and the same urban route is available to them if the site is designated.
